Who is Harrow District Nursing Service for?
This service is for people 18 and over who are unable to access nursing services from their GP practice. This could be either because they are housebound or due to certain medical conditions. They must be registered with a Harrow GP.
What does the service provide?
The Harrow District Nursing Service provides domiciliary nursing assessment, treatment and self-care support.
The service works closely with local hospital care providers and Harrow social services to ensure that service users are supported to remain in their home, to facilitate discharge from hospital and to prevent unnecessary hospital admissions.
The service also works closely with GP practices and specialist community nursing services. These include tissue viability, diabetes specialist nursing, end of life care services and coronary heart disease services to ensure that holistic medical needs of patients are met.
How do I access the service?
Referrals can be made by GPs and other health practitioners.
The service is available 24 hours, seven days a week.
The service is provided by Central London Community Health Care Trust (CLCH).
