Who is the Community Cardiology Service for?


Young people and adults aged 16 and over who are registered with a Harrow GP.

What does the service provide?

The service provides comprehensive, flexible and integrated evidence-based cardiac services in the community that meet local and national priorities and guidelines.

It aims to:

  • Reduce cardiovascular morbidity and mortality.
  • Provide evidence-based care that will meet the diverse needs of the local population.
  • Promote and implement good practice.
  • Enhance knowledge and skills in the effective management of cardiac disease.
  • Improve and enhance the standard of care.
  • Promote equitable access and ensure prompt diagnosis and treatment for those with or with suspected cardiac diseases.
  • Prevent unnecessary and inappropriate hospital admissions and prolonged length of stay.
  • Promote healthy living to reduce and prevent cardiac disease progression.

The service provides a comprehensive cardiac and diagnostic service that meets diverse needs. This includes:

  • Cardiac diagnostic service for patients who are non-acute and stable, but suspected of cardiac disease.
  • Cardiac rehabilitation programme for patients who have received cardiac surgery or at risk of cardiac event.
  • The heart failure service, for patients who have a left ventricular systolic dysfunction, advanced heart failure requiring end-of-life support, difficult to manage heart failure or multiple hospital admissions.

How do I access the service?

Referrals to the service are accepted from GPs.
The service is available from Monday to Friday, 9am to 5pm
The service is provided by London North West Healthcare NHS Trust (LNWHT)
