Who is Diabetes Specialist Nursing Service for?
Adults aged 18 years and over who are registered with a Harrow GP.
What does the service provide?
The service supports patients to manage conditions and improve patient outcomes.
The Tier 3 service provides diabetes intermediate care, which is delivered by consultants who work on a weekly basis at the Pinn Medical Centre, Alexandra Avenue Clinic, and Northwick Park Hospital.
The Tier 2 service provides diabetes clinics, which aim to improve patient outcomes and reduce risk of developing diabetes-related complications associated with poor control, such as supporting patients to control high blood pressure. Each GP Practice has a designated Diabetes Specialist Nurse.
The service also provides structured education programmes, including Diabetes XPERT (for Type 2 diabetes) and DAFNE (for Type 1 diabetes).
How do I access the service?
Referrals can be made by health professionals including GPs, practice nurses, dieticians, Allied Health Professionals, consultants, district nurses, or specialist nurses (e.g. palliative care nurses).
Patients can also refer themselves and contact the service directly.
The service is available from Monday to Friday, 9am to 5pm
